Hospital employment in IndianaWhat can you expect from an employer of choice? Our excellent benefits package is just the start. Employees who meet the eligibility requirements become eligible for benefits sixty days following their first day of employment.

Health Insurance
Logansport Memorial Hospital offers a choice of three health plans through SIHO Insurance Services:

  • Tier 1: $1,000 annual deductible
  • Tier 2: $500 annual deductible
  • Tier 3: $350 annual deductible

Dental Insurance
Dental insurance is also provided through SIHO Insurance Services, and there are three plans to choose from. Two annual exams and cleanings, X-rays and sealants for children under age 15 are covered at 100%. Partial benefits are also available for orthodontia, crowns, bridges and dentures.

Vision Insurance
Vision insurance is provided through the VSP Network. One eye exam is covered annually at 100% for covered members,and employees also enjoy discounts on eyeglasses, contact lenses and other vision services simply by being a member of the VSP Network.

Life Insurance
Basic Life Insurance is provided by Logansport Memorial Hospital in the amount of one (1) times your annual salary up to $100,000. Supplemental Life Insurance is available in increments of $1,000 with a minimum requirement of $10,000 up to a maximum of $500,000 or five (5) times your annual salary. You will also have the opportunity to provide life insurance for your spouse and/or dependents by purchasing additional life coverage with convenient payroll deductions.

Disability Insurance
Logansport Memorial Hospital provides you with Short Term (STD) and Long Term Disability (LTD) insurance to replace a portion of your income if you become disabled and can no longer work.

Flexible Spending Account Program
A great way to save on your health care and dependent care expenses is by taking advantage of the Flexible Spending Account (FSA) program offered at Logansport Memorial Hospital. By participating with an FSA it allows you to save on taxes by using pre-tax money to pay for health care costs and/or dependent care costs.

  • Health Care FSA: up to $5,000 pre-tax.
  • Dependent Care FSA: up to $5,000 pre-tax if head of household or a joint tax return and $2,500 maximum for married, separately filed tax returns.

Retirement and Savings
Logansport Memorial Hospital offers retirement benefits including a 403(b) match program and a pension program through ING Financial Advisers.

  • Match dollar for dollar up to 2% of your annual salary.
  • Contributes 3% of your annual salary automatically into a pension program.

Time off Benefits
