Health Insurance

Logansport Memorial Hospital offers a choice of two affordable health care plans through SIHO Insurance Services:

  • A traditional health insurance plan
  • A high-deductible health insurance plan with an optional health savings account

As an employee, you have the option of setting up a flexible spending account (FSA) to use pre-tax money to pay for health care costs for yourself and your dependents: 

  • Up to $5,000 maximum pre-tax if you are the head of household or file your tax returns jointly
  • Up to $2,500 maximum pre-tax if you are married but file your tax returns separately

You are also eligible to receive discounts on health insurance premiums if you participate in a health risk assessment. 

Dental Insurance

We provide two options for dental insurance through SIHO Insurance Services. Each year, for each individual on your plan, two exams and cleanings, X-rays, and sealants (for children under age 15) are covered at 100%. 

Partial benefits are also available for orthodontia, crowns, bridges, and dentures.

Vision Insurance

Logansport Memorial Hospital offers vision insurance through the VSP Network. Each year, for each individual on your plan, one eye exam is covered at 100%. 

Our employees can take additional membership discounts on eyeglasses, contact lenses, and other vision services.

Life Insurance

We provide basic life insurance to all employees in the amount of one times your annual salary, up to $100,000. 

Supplemental life insurance is available in increments of $1,000 with a minimum requirement of $10,000, up to a maximum of $500,000 or five (5) times your annual salary.  

Employees also have the opportunity to purchase additional life insurance to cover your spouse and/or dependents through convenient payroll deductions.

Disability Insurance

Logansport Memorial Hospital proves short-term and long-term disability insurance to replace a portion of your income if you become disabled and can no longer work. 

Retirement and savings

Logansport Memorial Hospital offers competitive retirement benefits, including a 403(b) match program through Voya Financial Advisers.

Vacation and time-off
