What is the patient portal?
Logansport Memorial Hospital’s HealtheLife patient portal offers you secure, online access to certain information in your electronic health record, including upcoming appointments, prescriptions, lab and imaging test results, immunizations, and discharge instructions.
Is there a fee associated with using the patient portal?
No. The patient portal is a free service for all Logansport Memorial Hospital patients.
I don’t remember my username and/or password. What do I do?
You can reset your password by yourself, by clicking the “Forgot Password” link and walking through the steps. If you continue to have problems, please contact the Cerner support line at (877) 621-8014.
What is CernerHealth?
CernerHealth is the company we use to securely manage your electronic health record. Logansport Memorial’s HealtheLife patient portal is a customized CernerHealth solution that enables you to interact with your medical records while keeping your health information safe.
What you can do with the patient portal
I was a Logansport Memorial Hospital patient in the past. Can I see my medical record for previous visits through the patient portal?
No, your historical medical records are not available through the Portal. Your records are available for visits made after July 20, 2020. If you need a copy of the previous records, you can obtain copies from the hospital’s Medical Records department.

How soon can I see my test results or medical information in the patient portal?
Your visit information should be available almost immediately. Test results from labs or imaging procedures should be available in no more than 36 hours. Some information may have an additional delay.
Accessing Health Records
Who else can access my Logansport Memorial HealtheLife patient portal account?
Only you have access to your patient portal account, unless you want to grant access to someone else. A separate email address is required for each person and each patient portal account.
Note: If you don’t own or control the computer you’re using to access your account, turn on private browsing to protect your personal health information.
Can I access my child’s or parent’s health information?
Yes. Parents can obtain health information for children under the age of 18, or if you are a patient’s caregiver. Call the Medical Records department at (574) 753-1390 to request access as a proxy.
Please know that as soon as your child turns 18, your proxy will no longer be in effect and they will need to create their own account. For caregiver-patient proxy requests, you will be required to provide additional documentation.
I am a power-of-attorney/legal guardian for my loved one. How do I sign them up?
Simply bring in the paperwork stating that you have power-of-attorney or legal guardianship, along with a photo ID of yourself to the office visit for your loved one. Our staff will assist you in getting them enrolled.
Surprise Billing
When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing.
