Health Insurance

Logansport Memorial offers a choice of two health plans through SIHO Insurance Services - one Traditional Plan and a High-Deductible Health Plan (HDHP) with a Health Savings Account (HAS) option.

Dental Insurance

Dental insurance is also provided through SIHO Insurance Services and there are two plans to choose from. Two exam and cleanings, X-rays, and sealants for children under age 15 are covered at 100%. Partial benefits are also available for orthodontia, crowns, bridges, and dentures.

Vision Insurance

Vision insurance is provided through the VSP Network. One eye exam per member is covered at 100% and employees also enjoy the discounts on eyeglasses, contact lenses, and other vision services simply by being a member of the VSP Network.

Life Insurance

Basic Life Insurance is provided by Logansport Memorial Hospital in the amount of one (1) times your annual salary up to $100,000. Supplemental Life Insurance is available in increments of $1,000 with a minimum requirement of $10,000 up to a maximum of $500,000 or five (5) times your annual salary. You will also have the opportunity to provide life insurance for your spouse and/or dependents by purchasing additional life coverage with convenient payroll deductions.

Disability Insurance

Logansport Memorial Hospital provides you with Short Term (STD) and Long Term Disability (LTD) insurance to replace a portion of your income if you become disabled and can no longer work.

Flexible Spending Account Program

A great way to save on your health care and dependent care expenses is by taking advantage of the Flexible Spending Account (FSA) program offered at Logansport Memorial Hospital. By participating with an FSA it allows you to save on taxes by using pre-tax money to pay for health care costs and/or dependent care costs.

Health Care FSA up to $5,000 pre-tax

Dependent Care FSA up to $5,000 pre-tax if head of household or a joint tax return and $2,500 maximum for married, separately filed tax returns.
