Logansport Memorial Hospital cannot provide emergency medical treatment or services to children without parental or guardian consent. Please fill out a minor consent form in case your child has a need for care while you are not present to provide consent because of your hospital stay.

During your hospital stay

During your recovery, we want you to feel as safe and comfortable as possible. All of Logansport Memorial Hospital’s inpatient rooms are private, and we provide 24-hour security for patient and visitor safety.

Patient meals

Patient meals are served from 6 a.m. to 6 p.m. We offer a restaurant-style menu as well as an on-site cafeteria for you and your guests.

Telephones

Each patient room has a telephone for receiving calls, except in the intensive care unit. Your family and friends may call you in your room through the main hospital line—(574) 753-7541 or toll-free at (800) 243-4512—or by dialing the number on the phone at your bedside. You can make local calls at any time by dialing 9 plus the number. You can make long-distance calls by calling collect or charging the calls to your credit card.

A telecommunications device for the deaf (TDD) and telephone amplification devices for the hearing impaired are available for incoming and outgoing calls. Additionally, Logansport Memorial Hospital has a direct line to the TDD—(574) 737-2099—from outside of the hospital.

Cell phones may be used in all common areas of the hospital including the cafeteria, lobby, conference rooms, and administrative areas. In order to protect patients and visitors from privacy issues, cell phones may not be used in patient-care areas. Please remember privacy, courtesy, and confidentiality when using your cell phone.

After your hospital discharge

Patients are always eager to get back to their house to finish the recovery process. Once you’re back home and feeling normal again, you might have some questions about paperwork related to your time being admitted to the hospital. We can help.

Request your Medical Records

To get a personal copy of your medical records or to have your records forwarded to another party, we need you to complete an “Authorization for Disclosure of Protected Health Information” form. You can download a copy of the form in English or Spanish below, fill it out, and bring it with you to meet with our representative privately. To find the Medical Records office, please stop at the Information Desk in the Main Lobby and ask for assistance in requesting your medical records. You also need to bring your government-issued ID so that we can verify your identity before releasing the records.
